RN Care Manager

Job Description

The RN Care Manager is responsible for managing the patient care continuum from admission through discharge at UAMS. This role involves demonstrating expertise in the RN scope of practice and adhering to current state regulations. The manager collaborates with a multi-disciplinary team, applying scientific evidence to ensure best practices and effective care. Key duties include coordinating the discharge planning process, documenting all communication and concerns related to care coordination, and communicating with physicians regarding level of care or admission status when criteria are not met. Additionally, the role involves identifying patients/families with psychosocial and/or non-medical discharge planning needs and contributing to Performance Improvement and Evidenced Based Practice/Research processes to enhance patient outcomes.

Qualifications

1. Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree. 2. Three (3) years RN experience with current BLS certification. 3. Valid Arkansas Registered Nursing License. 4. National Nursing Certification preferred; BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification.
