Join Stellus Rx as a Population Health Pharmacist and play a pivotal role in enhancing healthcare outcomes for communities. This role involves developing, implementing, and evaluating population health initiatives focused on medication management, adherence, and therapeutic outcomes optimization. Utilize health data analytics to pinpoint trends and opportunities for intervention. Collaborate with healthcare providers to ensure appropriate medication selection, educate patients on medication use, and actively participate in quality improvement projects to advance evidence-based practices. Provide leadership within the pharmacy team and contribute to departmental goals, ensuring compliance with pharmaceutical regulations.

Qualifications

1. Doctor of Pharmacy (PharmD) degree from an accredited institution. 2. Active pharmacist licensure in both your state of residence and TX in good standing. 3. Advanced training or certification in population health management or a related field (preferred). 4. Strong analytical and data interpretation skills. 5. Experience in pharmacy practice, preferably in a population health or managed care setting. 6. Excellent communication, collaboration, and leadership abilities. 7. Knowledge of healthcare regulations, policies, and quality improvement methodologies.
