Job Description

Join our team as a Clinical Licensed Vocational Nurse (LVN) and play a vital role in providing remote patient care. You'll connect with patients via phone, addressing their health concerns and coordinating with providers to ensure timely and effective treatment. Your responsibilities include: 1. Conducting outbound phone calls to check in on patients (70-90 calls per day). 2. Handling inbound phone calls, routing them based on clinical urgency. 3. Monitoring and responding to Remote Patient Monitoring (RPM) alerts. 4. Performing monthly wellness assessments and comprehensive chart reviews. 5. Accurately documenting all patient interactions in our clinical platform. 6. Meeting or exceeding performance metrics related to care quality and patient engagement. 7. Maintaining compliance with company policies and regulations. 8. Completing other duties as assigned, including AI fluency.

Qualifications

To excel in this role, you'll need: 1. Active LVN/LPN license. 2. Proficiency with computers, EMRs, and telehealth tools. 3. Strong communication and organizational skills. 4. Bilingual proficiency in English and Spanish. 5. At least 1 year of nursing experience (RPM, telehealth, or chronic care experience is a plus).
