Clinical CCM LVN, Bilingual
Job Description
Provide thorough assessment calls to gather comprehensive details about patients' health. Identify and escalate patient requirements, such as refill requests and appointment scheduling, connecting individuals with community resources. Formulate comprehensive care strategies grounded in patient evaluations. Conduct Behavioral Health Integration (BHI) evaluations, addressing the mental health facets of patient care. Uphold adherence to company protocols and relevant regulations. Additional duties as assigned. AI fluency is a non-negotiable requirement.
Qualifications
1. Possess an active LPN/LVN Compact License. 2. Exhibit strong communication and organizational skills. 3. Demonstrate bilingual proficiency in English and Spanish. 4. Possess proficiency in computer skills, EMRs, and telehealth tools. 5. Display passion for patient care, relating with empathy and compassion. 6. Have a passion for patient teaching. 7. Have at least 1 year of nursing experience (RPM, telehealth, or chronic care experience is a plus).
