CCM Intake MA, Bilingual

Job Description

We are hiring a Bilingual CCM Intake Medical Assistant. In this role, you will support the Chronic Care Management (CCM) team by conducting patient assessments and Chronic Care Planning Assessment (CCPA) calls. You'll handle clerical tasks, document interactions in the EHR, and ensure compliance with regulations. This role involves coordinating communication between patients, caregivers, nursing staff, and providers, contributing to improved patient care and outcomes. The position requires a proactive individual passionate about patient well-being and effective healthcare delivery.

Qualifications

1. Active MA license is a must. 2. Strong communication skills are required. 3. Must be fluent in both English and Spanish. 4. Empathy and compassion for patient care is a must. 5. Passion for patient teaching is required. 6. Minimum fiber internet speed of 50 Mbps download / 10 Mbps upload is a must. 7. Hardwired internet connection is required. 8. Speed test submission is required during the offer process. 9. A private, HIPAA-compliant workspace is required.
