Case Manager, Registered Nurse

Job Description

Seeking a dedicated Care Manager Registered Nurse to coordinate comprehensive care for members managing chronic medical and behavioral health conditions. This role involves conducting thorough assessments to understand member needs, creating and implementing personalized care plans, advocating for essential services, and working closely with an interdisciplinary care team. The position requires meticulous documentation of all care management activities.

Qualifications

1. Must possess an active and unrestricted Registered Nurse (RN) license in the state of NY. 2. Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams is essential. 3. A minimum of 3 years of nursing experience is required. 4. Candidates should have at least 2 years of experience in case management, discharge planning, or home healthcare coordination. 5. Experience in providing care management for Medicare and/or Medicaid members is highly desirable. 6. Bilingual skills, particularly English-Spanish, are preferred.
