Job Description

Join our multidisciplinary team at RHD South Dakota, dedicated to providing holistic, values-driven services. As a Case Manager, you'll: - Collaborate in developing and reviewing recovery plans with program participants, clinical staff, and other stakeholders. - Build and maintain relationships with community agencies, connecting participants with available resources. - Facilitate team processes for the initial and annual development of Individual Service Plans (ISPs). - Monitor ISPs, document results, and intervene as necessary. - Report and review Critical Incident Reports (CIR) according to protocol. - Participate in Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews. - Focus on Personal Outcome Measures. - Apply knowledge of Charting the Life Course framework and Person-Centered Thinking/Planning. - Evaluate client needs to ensure community success. - Connect clients with needed programs and supports. - Complete required documentation, including data input and report creation.

Qualifications

1. Bachelor’s degree and one year of experience OR equivalent education/experience. 2. Intermediate computer skills. 3. Ability to establish a HIPAA-compliant home office. 4. Valid driver’s license.
